Well there's 2 versions. 1) He refused and 2) he was working. I think it would have been interesting with Ian and a different Doctor.

Actually, many years ago I did a crossover story where Ted was really Ian and his first wife Doris was Barbara. He forced the 4th Doctor to cure his tumour but got killed anyway by a rogue Cyberman that got into the Tardis at the bowls match.
